Hosmer also hit a solo homer and Wilson Betemit added a game-tying double in the eighth for the Royals, who snapped a seven-game road losing streak that included Tuesday's series-opening 3-1 setback.
Joakim Soria (3-0) allowed an RBI single in the bottom of the 10th to Curtis Granderson but still picked up the win. Louis Coleman retired the side in order in the bottom of the 11th for his first save.
Granderson went deep and Jorge Posada finished with two hits and an RBI for the Yankees, whose two-game win streak was halted.
Buddy Carlyle (0-1) was charged in defeat for two runs in just over one relief inning.
Up 3-2 in the 10th, Soria walked Russell Martin to start the inning. He was sacrificed to second, moved to third on a groundout then scored on Granderson's hit.
In the 11th, Chris Getz worked a leadoff walk against Carlyle, then Luis Ayala allowed a sac bunt from Alcides Escobar and an infield single from Jarrod Dyson. Dyson stole second then Melky Cabrera was intentionally walked before Hosmer lofted a fly to center to bring in Getz.
"Knowing Getz was on third and can run, I was just trying to get it in play," Hosmer said.
Coleman had an easy bottom portion to wrap up the win.
"Right now we're struggling offensively," Yankees manager Joe Girardi said. "We're better than this."
Posada singled in Robinson Cano in the second, and Granderson launched his 12th homer of the season to center to begin the third.
Hosmer belted his first career homer to right to start the fourth, and in the eighth, Cabrera walked and eventually came around to tie the game on a Betemit single.
Jeff Francoeur's double plated Hosmer in the top of the 10th to give Kansas City a 3-2 edge, and Soria was one out away from victory before Granderson forced another inning.
KC starter Vin Mazzaro, recalled from Triple-A Omaha prior to the game, allowed two runs and six hits with four walks and two strikeouts in four innings.
His counterpart, A.J. Burnett lasted seven frames and yielded only Hosmer's homer while fanning six. He also walked five.
