Jabari Parker, Giannis Antetokounmpo and Khris Middleton each scored 14 points for the Bucks, who snapped a three-game losing streak.
Dwyane Wade scored a game-high 28 points for the Heat and Chris Bosh had 21 in their third straight loss.
Jerryd Bayless scored 11 of his 12 points in the third quarter for the Bucks, who turned a tight game into a nine-point lead going into the fourth with a 13-2 run that came mostly at the foul line.
Bayless hit six of Milwaukee's nine free throws during the spurt, which made it 80-71.
The Bucks then outscored Miami 29-14 in the fourth, leading by double-digits all the way after Middleton's jumper to open the quarter.
They shot better than 64 percent in the quarter and over 56 for the game.
"Our guys shared the ball and knocked down shots," said Bucks coach Jason Kidd.
Miami had just one field goal in the final 6 1/2 minutes of the game, but Wade blamed the loss on what happened at the other end, saying, "We're just not a good defensive team right now."
There were other problems, too. Milwaukee out-rebounded the Heat 45-20, with a 23-9 margin in the second half.
"They didn't miss that many shots," Wade laughed. "When you're hitting on 56 percent, there aren't that many rebounds out there."
Wade had 11 points in the first quarter and the Heat shot 55 percent, leading by as many as six. Bosh's 3-pointer pushed it to 11 under a minute into the second, but the Bucks countered with an extended 20-8 run led by seven points from Jared Dudley.
Wade scored the last six Heat points in the quarter, giving them a 54-50 lead at halftime.
