James scored 17 points in the fourth quarter - two more points that the entire Trail Blazers' team - and made a reverse layup with 0.9 seconds to play for the victory.
The Cavaliers have won five straight home games. Portland has won two straight, but has now become a longshot to make the playoffs in the loaded Western Conference.
"We don't feel like we're out of the playoffs yet," Trail Blazers power forward LaMarcus Aldridge said. "I think we're trying to finish this season off good. We know if we don't make the playoffs we still want to finish strong."
