Aceves is starting in place of John Lackey, who had to leave Saturday's contest with a biceps strain. Aceves has appeared in two games out of the bullpen for the Red Sox this season and has allowed five runs in 4 1/3 innings.
"One of the reasons we talked so positively about Alfredo is the versatility he does have, and we'll get a look at that (Thursday) night," said Red Sox manager John Farrell.
This will be his first start since June 21, 2011.
Baltimore, meanwhile, will counter with righty Chris Tillman, who was roughed up by Minnesota his first time out. Tillman did not get a decision in that one, but yielded five runs and seven hits in 3 2/3 innings.
"I was real pleased with the stuff he featured, just couldn't get into a consistent rhythm," Orioles manager Buck Showalter said after Tillman's last outing. "He'll be better."
Tillman has faced the Red Sox five times and is 1-1 with a 2.88 ERA. In his only two starts at Fenway, though, Tillman has allowed just one run over 11 innings.
Baltimore evened this series on Wednesday, as Manny Machado's three-run homer capped a five-run ninth inning in the Orioles' 8-5 come-from-behind win.
"It was a huge win for us," Machado said. "To come out with the victory is just going to give us the big boost that we needed to get this show rolling."
Joel Hanrahan (0-1) was tasked with getting the final three outs for Boston, which carried a 5-3 lead into the ninth.
Chris Davis opened the frame with a solo home run to center field. Hanrahan, who was looking to nail down his 100th career save, retired the next two batters before everything unraveled.
Ryan Flaherty singled and consecutive walks to Nolan Reimold and Nate McLouth loaded the bases. The tying run scored on a wild pitch and Machado put Baltimore in front with his drive off Hanrahan to the Green Monster seats.
"You've got your closer in the game, the ballgame locked up, it's tough," Red Sox catcher Jarrod Saltalamacchia said. "But I still feel that Joel's got some of the best stuff I've ever caught."
Boston went 1-2-3 in the bottom of the ninth, with Jim Johnson earning his third save of the season. Darren O'Day (1-0) was awarded the victory after pitching a perfect bottom of the eighth.
Baltimore dominated the Red Sox last season, taking 13 of the 18 matchups.
